Cave Report Forms.
The ICS has recently up-graded the Report Form
System. There are 2 reports you may choose from to submit a Cave Report Form.
A New Cave Report is for a New Discovery,
or, a cave you believe has never been submitted to the ICS. This report form
requires
many of the data fields to be completed.
A Correction Report is to up-date
information on an already known cave, whether it be all information, or, just
partial submission of information (location only, Landowner only, ect.). This
form is much less restrictive in the information required to submit.
PLEASE COMPLETE AS MUCH INFORMATION AS POSSIBLE... THIS WILL HELP THE COUNTY
DIRECTOR BETTER
IDENTIFY THAT A REPORT IS CORRECT.

NOTE: If a County Director is unable to verify what you are reporting, or, what cave it may be... the report will be discarded.
It is the intent of the ICS to record as much information as accurately as possible. Please review the data which is requested, and complete as much information as you possibly can, as this helps to maintain a complete and accurate accountability for every Indiana Cave.
There are many fields on the report form which is required to be completed, as this ensures accuracy of your report when being added to the Database.
